I, ___ (___), hereby give my consent for my child, ___ (___) to participate in the field trip on 8th August 2025, organized by Kids’ House Development Sdn. Bhd. to Tropical Fruit Farm.
I, acknowledge that the teachers and staff will actively supervise the trip and that Kids’ House has taken all reasonable safety measures to ensure a secure and enjoyable experience for the children. I understand that while safety precautions will be in place, there is always a level of risk involved in any outdoor activity.
By signing this form, I release Kids’ House Development Sdn. Bhd., its teachers, staff, and volunteers from any liability arising from accidents or incidents that may occur during the field trip, except in cases of gross negligence. I also accept responsibility for any costs associated with my child’s participation in the Field Trip.
